Our Story

Dr. Joshua Reischer, a board-certified internal medicine physician, spent years on the front lines of care, watching hours every day disappear into collecting, documenting, and organizing patient information instead of caring for patients. He didn’t see this as a documentation problem. He saw it as an access problem. Patients struggled to prepare. Staff struggled to keep up. Providers were buried in fragmented data before the visit even began.

At YC Startup School, Josh met Aaron Rau, a serial entrepreneur who had built software platforms used by organizations like Goldman Sachs and Medicare. Together, they shared a simple conviction: healthcare needed intelligent digital teammates, not more tools.

That belief became Health Note. Today, Health Note powers millions of patient interactions nationwide, deploying AI-driven Access and Intake Assistants that help patients prepare earlier, guide them to the right care, and ensure clinicians walk into visits with clarity instead of chaos.
